How to be Referred to Seating and Mobility Services?Seating and Mobility Services provides assessment and prescriptions of seating and mobility devices to individuals up to age 18 who live in Waterloo Region and Wellington County. Services are provided at our Waterloo, Cambridge, and Guelph sites. To see site locations click Contact Us. Criteria for Referrals
- You have a child or youth (0-18 yr of age) with a developmental or physical disability
- who has a seating and/or mobility need
- and you reside in the Region of Waterloo or Guelph-Wellington
Seating and Mobility Services Referral Process
New Referrals
- New referrals need to be made by an Occupational Therapist (OT)
- The Occupational Therapist can make a referral by contacting KidsAbility Health Records at (519) 886-8886 x1214 or 1-888-372-2259 x1214
Re-Referrals
- If your child has previously been seen by Kidsability Seating and Mobility Services please contact KidsAbility Health Records at (519) 886-8886 x1214 or 1-888-372-2259 x1214
- You may re-refer to Seating and Mobility Services until your child is 18 years of age and an Occupational Therapist referral is not required for subsequent referrals. For more information click on When Should I Return to Seating Clinic (SCL)

For adult Seating and Mobility Services you may contact ADP for a list of ADP authorizers in your area. |
